Identify the Fetus Receiving Care With Your ICD-10 Code

Question: I’m a little confused as to which twin I am identifying with this specific ICD-10 code. If I have a twin pregnancy with fetal demise with retention of the second twin, in the third trimester. Is code O31.23x2 correct? Do I identify the twin that died (fetus #2) or should I identify the fetus that is the continuing pregnancy (fetus #1), because that is the description of the code?

Answer: Per ICD staff, you should assign the seventh digit based on the fetus that is still alive and receiving care. Therefore, if fetus #1 is still alive, then you should report O31.23x1 (Continuing pregnancyafter after intrauterine death of one fetus or more, third trimester, fetus 1).
