Question: If a patient presents with his own Zoladex (J9202), what should we use for the administration?

Oklahoma Subscriber Answer: You should report Zoladex administration (90782, Therapeutic, prophylactic or diagnostic injection [specify material injected]; subcutaneous or intra-muscular) only when the oncologist or staff furnishes and provides the drug. If a staff member administers Zoladex, the physician must supervise the procedure.
