Answer: You submit 1 unit of code J9035 (Injection, bevacizumab, 10 mg) for every 10 mg of bevacizumab.

What is bevacizumab? Bevacizumab is a monocloncal antibody that binds to the vascular endothelial growth factor and inhibits its activity. It prevents the formation of new vessels.

The code J2430 (Injection, pamidronate disodium, per 30 mg) is for pamidronate disodium. This is used to treat hyperkalemia due to cancer, Paget's disease, osteolytic bone lesions of multiple myeloma, and metastasis of breast cancer.

The code J9041 (Injection, bortezomib, 0.1 mg) is for bortezomib which is an anti-neoplastic agent. Bortezomib is a modified form of boronic acid.

The code J9190 (Injection, fluorouracil, 500 mg) is for flurorouracil (5 FU). This fluorinated analogue of uracil is used as an anti-neoplastic agent.
