Question: If my oncologist administers Faslodex in two sites, should we bill it 96402 and 96402-59?

Answer: You should bill the Faslodex administration in two different line items as:

  • 96402 (Chemotherapy administration, subcutaneous or intramuscular; hormonal anti-neoplastic; Distinct procedural service)
  • 96402-59 (... Distinct procedural service)

You should place modifier 59 on the second 96402 code to identify that the physician performed the administration of the second injection in a separate site from the first injection.
