Question: What services should I include in 77427?

Answer: Code 77427 (Radiation treatment management, five treatments) covers the supervision and care of the patient during radiation therapy. Translation: Most of the services provided by your radiation oncologist during this period fall under 77427.

You should include review of port films, review of dosimetry, dose delivery, and treatment parameters, and review of patient treatment set-up in one unit of 77427. You should also include assessment of the patient in the radiation treatment management code along with any changes to the plan.

Don't miss: You may hear providers refer to these services as -weekly treatment management,- but the descriptor for 77427 specifically refers to -five treatments.- Be sure you report the code once for every five treatments rather than once per calendar week of treatment.

You may code three to four leftover fractions (radiation therapy treatment units) as an additional unit, but don't report 77427 or 77431 (Radiation therapy management with complete course of therapy consisting of one or two fractions only) for one or two leftover fractions.
