Reader Question:

Select 23405 or 29999 for Tenotomy

Question: Our surgeon documented a biceps tenotomy. How should I report this?

Wisconsin Subscriber Answer: If the surgeon performed the tenotomy as an open procedure, you should report 23405 (Tenotomy, shoulder area; single tendon). If he documented an arthroscopic tenotomy, you should report the unlisted-procedure code 29999 (Unlisted procedure, arthroscopy).

Unfortunately, CPT does not include a more specific code for arthroscopic biceps tenotomies. Include a letter with your claim describing the arthroscopic procedure, and compare it to another procedure of similar difficulty (such as 23405) to determine your reimbursement amount.
