In-office Test

Question: When I perform an in-office test, can I just document that the test is normal, or must I place a copy of the printout in the notes? Specifically, Im referring to tympanograms, the Welch Allyn SureSight vision screener and the Audiopath hearing screener.

Answer: For medical and legal reasons, its always a good idea to include a hard copy of the test with the chart.

If the only record in the chart is the physicians interpretation of the test, and there is a future problem in the area tested, he or she has no defense. A printout also provides proof to insurers that the test(s) was performed. All of these machines have printouts its just a matter of filing them.

The codes for the procedures you refer to are 92567 for the tympanogram, 92015 for the SureSight, and 92587 or 92588 for the Audiopath.
