Discover Skin CPT® Biopsy, Dx Codes

Question: We were frequently billing the old punch biopsy code 11110 with ICD-10 code G62.9. Now, when we bill the new punch biopsy code 11104 with G62.9, Medicare, denies the claim based upon the diagnosis. As far as we can tell, there are no “G” diagnosis codes acceptable for this new skin biopsy code. Many of the patients are diabetic. Can you help us?

Answer: Code G62.9 (Polyneuropathy, unspecified) represents unspecified polyneuropathy, and 11104 (Punch biopsy of skin (including simple closure, when performed); single lesion) needs a more specific diagnosis code of the underlying cause such as codes E08.4- (Diabetes mellitus due to underlying condition with neurological complications) through E08.49- (Diabetes mellitus due to underlying condition with other diabetic neurological complication). Although these codes would only apply if the patient is a diabetic. You should definitely talk to your physicians and make sure they include any symptoms the patients are experiencing in the documentation.
