Reader Question:

Know Which Modifiers to Use for Toe Modifications

Question: Can 28112 only be billed once or can it be billed multiple times? If you are able to bill this procedure multiple times, would an anatomical modifier need to be used? My provider performed this procedure on the second, third, and fourth digits so would I use modifier T6, T7, T8, or just modifier 59?

Answer: You can bill 28112 (Ostectomy, complete excision; other metatarsal head (second, third or fourth) multiple times using modifier 59 (Distinct procedural service) when more than one metatarsal is resected; but using T modifiers for each metatarsal resection, like T7 (Right foot, third digit) for example, is also recommended to avoid a claim denial.
