Peak Flow Rate Challenges Coders

Question: How should I report a peak flow measurement taken in the office prior to an allergy shot using a peak flow device?

Answer: CPT offers no distinct code for reporting peak flow rate measurements. Peak flow rate is a simple test that has a "B" status on Medicare's Physician Fee Schedule, so Medicare always bundles the peak flow rate measurement into payment for other services. Therefore, you cannot bill separately for peak flow and receive payment. You'll simply report the E/M service your pulmonologist provides.

Tip: If your pulmonary function technologist conducts all of the other measurements of spirometry, you should report the appropriate spirometry code.
