Pick out Primary Diagnosis Z Codes Easily

Question: I really enjoy the coding quizzes that test my knowledge. In Pulmonology Coding Alert, Volume 24, Issue 7, the first coding quiz answer involved a Z code as the primary diagnosis code. Is there a way to know what Z codes should be sequenced first?

Answer: ICD-10-CM Official Guideline, section I.C.21.c.16 lists 19 codes or code categories that can be reported as first-listed diagnoses, “except when there are multiple encounters on the same day and the medical records for the encounters are combined.”

Examples of the codes and categories listed in this section include:

  • Z00.- (Encounter for general examination without complaint, suspected or reported diagnosis)
  • Z01.- (Encounter for other special examination without complaint, suspected or reported diagnosis)
  • Z34.- (Encounter for supervision of normal pregnancy)
  • Z52.- (Donors of organs and tissues)
