'Operative Field' Guides 75898 Coding

Question: Should I report 75898 once per study?

Answer: You typically may report 75898 (Angiography through existing catheter for follow-up study for transcatheter therapy, embolization or infusion) once per operative field per session, according to CPT Assistant (December 2007). Tip: Multiple vessels in the same extremity are in the same operative field.

Exception: The AMA doesn't include central nervous system or intracranial completion angiography in this "once per operative field" rule. When you do report more than one completion angiography on a claim, CPTAssistant recommends using modifier 59 (Distinct procedural service) on the second and subsequent code to indicate their distinct nature.

You also should be on the lookout for all-in-one codes, such as 37210 (Uterine fibroid embolization [UFE, embolization of the uterine arteries to treat uterine fibroids, leiomyomata], percutaneous approach inclusive of vascular access, vessel selection, embolization, and all radiological supervision and interpretation, intraprocedural roadmapping, and imaging guidance necessary to complete the procedure). CPT notes following this  code state that you should not report 75898 with 37210, which CPT created as a one-stop-shop for UFE.
