Urine Test

Question: Which CPT code should I use for a post-ejaculate urine test?

Answer:  For the office visit, bill an E/M code (99201-99215). Use 81015 (Urinalysis; microscopic only) for the test. Do not use codes for semen analysis. If a pre-ejaculatory urine microscopy exam is performed on the same day, code the first 81015, and the second 81015 with modifier -91 (Repeat clinical diagnostic laboratory test). Other modifiers, such as -59 (Distinct procedural service) and -76 (Repeat procedure by same physician), may be required by certain carriers. However, the tests are often performed on separate days, and no modifiers are required.
