I have a patient who was born premature (33 wks) who is at great risk for deafness/hearing loss. The patient needs audiology testing every 6 months until approx age 3yrs. My provider is asking what she should use as a diagnosis since I have drilled into her not to use OB and/or out come of labor codes. I cant find anything other than Z87.898 Personal history of other specified conditions - Factors influencing health status and contact with health services and its SO generic I hate to recommend it. Anyone know of anything else? No current conditions other than hx of premature birth.
 
You could use: P07.36 - Preterm newborn, gestational age 33 weeks, 0 days through 33 weeks, 6 days
