Check your documentation and claim against these questions: • What kind of test did the cardiologist perform? • Do you have a diagnosis listed on your payer's covered list? • What, if any, supplies did the cardiology practice provide? • Where did the cardiologist perform the test? • Was the cardiologist in the office suite? • Is the patient covered by Medicare or a commercial carrier?
