Score 0 if patient can ambulate everywhere without device or human assistance. Score 1 if patient ever needs a one-handed device but never needs more than that (including human assistance). Score 2 if patient needs a two-handed device on level surfaces and/or another person present on uneven surfaces or stairs. Score 3 if patient always needs another person present to ambulate (regardless of device or surface). Score 4 if patient is chairfast but able to wheel self independently. Score 5 if patient is chairfast but cannot wheel self independently. Score 6 if patient is unable to ambulate or be up in a chair.
