The end-stage renal disease demonstration program, announced by Centers for Medicare & Medicaid Services Administrator Tom Scully May 29 (see MLR Vol. 29, No. 22, p. 3) is official. CMS published a notice on the demo in the June 4 Federal Register. In other Register notices, the agency invited comments on a number of information collection initiatives, including: extension of the Medicaid drug rebate program form for manufacturers; extension of the Medicare managed care disenrollment form; a new collection, "Medicare Decisions and Your Rights," which fulfills Medicare+Choice practitioner's' obligation to inform enrollees of their "right to obtain a detailed notice regarding services;" and a new survey tool to gather feedback on the agency's Web site, www.cms.hhs.gov.
