Ovarian Drilling

Question: My physician did a laparoscopic cystectomy with right ovarian drilling for polycystic ovaries. What is the code for ovarian drilling?

Answer: The American College of Obstetricians and Gynecologists (ACOG) coding committee dealt with this issue in 1999 and recommended that the ovarian drilling be coded as 49322 (laparoscopy, surgical; with aspiration of cavity or cyst [e.g., ovarian cyst] [single or multiple]). The cystectomy would be coded as 58662 (laparoscopy, surgical; with fulguration or excision of lesions of the ovary, pelvic viscera, or peritoneal surface by any method). Some payers will consider 49322 bundled with 58662, so contact your carrier.
