Report 59025 and 76815 for Modified BPP

Question: My doctor performed what she calls a modified biophysical profile (BPP). The report is an amniotic fluid index (AFI) non-stress test (NST). Which CPT code(s) should be used?

Answer: You should report 59025 (Fetal non-stress test) and 76815 (Ultrasound, pregnant uterus, real time with image documentation, limited [e.g., fetal heart beat, placental location, fetal position and/or qualitative amniotic fluid volume], one or more fetuses). Remember to append modifier -59 (Distinct procedural service) to 59025 to prevent the payer from bundling these two codes into the BPPcode.

During the NST procedure (59025), the ob-gyn monitors the fetal heart rate using external transducers. NST differs from routine monitoring because the patient marks fetal movements on the minor strip, which the physician then interprets. CPT advises coders to report 76815 for the "quick-look" exams "limited to the assessment of one or more of the elements listed in 76815," such as qualitative amniotic fluid volume.
