Use Unlisted For This Vaginal Cuff Repair

Question: How should I report this? Procedure: “Exploratory laparotomy with repair of vaginal laceration and lysis of adhesions.” The laparotomy and lysis is 44005. I can’t find anything to use for the laceration repair. The vaginal cuff was lacerated at the top of the vagina (3cm laceration). Laceration was closed with a running lock stitch (12002), but it doesn’t seem right to me to use this code, because the patient was open. Patient had bleeding and pain after intercourse. This was not an accident, nor a repair after a surgery. What should I do?

Answer: The vaginal cuff repair is an unlisted procedure (58999, Unlisted procedure, female genital system [nonobstetrical]). You can compare the work to 12013 (Simple repair of superficial wounds of face, ears, eyelids, nose, lips and/or mucous membranes; 2.6 cm to 5.0 cm) as the cuff is more like a mucous membrane repair.
