All O Codes Demand Gestational Age

Question: If a patient says she has a complete AB, do we need to list the week’s gestation diagnosis code on top of the normal code in ICD-10? If so, do we select the weeks based on the U/S, or by what she stated was her last menstrual period?

Answer: In ICD-10, the gestational age is required for all codes that begin with the letter “O” and any code for an abortion would start with this letter (e.g., O03.4, Incomplete spontaneous abortion without complication) You assign the gestational age she was when she aborted based on your most accurate assessment. Calculation by ultrasound is considered more accurate that the LMP, but for very early abortions, you may not have this information so you then use the date of her last monthly period.
