Connecticut Subscriber

Answer: The diagnosis code for HPV is 079.4 (Human papillomavirus). If the patient has a Pap smear done and the test comes back with risk for HPV, you should use 795.05 (Cervical high-risk human papillomavirus [HPV] DNA test positive) or 795.09 (Other abnormal Papanicolaou smear of cervix and cervical HPV) for a low-risk with a positive DNA test.

For the HPV test, look at 87620 (Infectious agent detection by nucleic acid [DNA or RNA]; papillomavirus, human, direct probe technique), 87621 (... papillomavirus, human, amplified probe technique) and 87622 (... papillomavirus, human, quantification).
