Spotting and Cramping: What Diagnosis Code(s) to Report

Question: The provider documents a pregnant patient is having spotting and cramping. Is cramping a symptom of spotting? Do I just use O26.85-? Or do I code cramping R10.2?

Answer: Cramping is not a symptom of spotting as you can have spotting without it. Feel free to indicate both conditions. Most likely, you would want to report (O26.89-, Other specified pregnancy related conditions) for the cramping instead of R10.2 (Pelvic and perineal pain).
