Use 58145 for Transvaginal Myomectomy

Question: The ob-gyn performed a transvaginal myomectomy. He did not use an operative hystero-scope. Should I report 58561-52? Is this my best choice for coding?

Answer: No, 58145 (Myomectomy, excision of fibroid tumor[s] of uterus, 1 to 4 intramural myoma[s] with total weight of 250 grams or less and/or removal of surface myomas; vaginal approach) is the better choice because it is the vaginal approach code. 

You should not use 58561-52 (Hysteroscopy, surgical; with removal of leiomyomata; reduced services) because the more accurate code (58145) is available that describes the procedure your ob-gyn performed.
