New Medicare rules outlined in Medlearn Matters article MM4215 stipulate that both the consulting and requesting physician must maintain documentation in support of consultation services (99241-99255), including both a written request and reason for the consultation.

To ensure that requesting physicians are holding up their end of the deal, fax this handy sheet to their office with all the relevant information included.
