Pick Out Pyriformis Injection Codes

Question: One of our physicians is going to be doing a Pyriformis injection. What code should we use?

Answer: Look to your provider's documentation for the answer. If your provider schedules a pyriformis injection, this procedure may be a deep muscle injection or another term for a nerve injection. The pyriformis (piriformis) muscle is a deep muscle in the buttock lying diagonally below the large gluteus maximus muscle. It lies on top of the sciatic nerve. Hopefully, the documentation will "clearly tell" what he performed - if not, ask the provider. Potential codes are:

  • 20552 - Injection(s); single or multiple trigger point(s), one or two muscle(s)

  • 64445 - Injection, anesthetic agent; sciatic nerve, single

  • 64450 - Injection, anesthetic agent; other peripheral nerve or branch.
