Decipher Interstitial Lung Disease Diagnosis Coding

Learn how to identify the phases of ILD.

Coding for interstitial lung disease (ILD) is complex. There are so many groups of disorders contributing to the architecture of lungs and scar tissue inflammation that can disrupt the delicate fibers of the lungs. This complexity creates a challenge for providers and medical coders alike to accurately translate the clinical aspect into the universal language of ICD-10-CM diagnosis codes.

Understanding the challenges of coding for ILD is essential to accurately assess documentation when assigning appropriate codes.

Get to Know Interstitial Lung Disease

ILD is not a single disease, but rather a diverse group of around 200 disorders, which contribute to the structure of the lungs, scar, and inflame the delicate functions of the organs. Disorders that cause inflammation and/or scarring (fibrosis) of the lung tissue have been identified and linked to ILD, especially within the alveoli. When lung tissue becomes thickened and stiff, the ability to function becomes difficult and can impede the efficacy of oxygen transfer into the blood.

Over time, breakdown in the alveoli combined with fibrosis results in an increase in symptoms including shortness of breath (dyspnea), a dry persistent cough, fatigue, and potentially, in advanced cases, respiratory failure.

Define the Features of ILD

Causes of ILD can result from many different factors, including:

  • Autoimmune diseases like rheumatoid arthritis (RA), scleroderma, or lupus
  • Environmental antigens such as asbestos, silica dust, mold, bird proteins, or other inhaled irritants
  • Certain medications, including chemotherapy and heart medications, have been linked to ILD
  • Genetic factors
  • Unknown causes (idiopathic) where no specific cause is identified; the most common example is idiopathic pulmonary fibrosis (IPF)

Access Diagnosis Codes for ILD

To accurately translate clinical aspects of ILD into relatable diagnosis codes, look to Chapter 10 of the ICD-10-CM code book. Chapter 10 includes the J codes that cover respiratory system diseases. It is crucial to understand block notes and exclusion guidelines listed at the beginning of this chapter. From a clinical aspect of diagnosing ILD, doctors may use:

  • Medical history and physical examination
  • Pulmonary function tests (PFTs)
  • High-resolution CT (HRCT) scans
  • Blood tests
  • Bronchoscopies or lung biopsies

Progression of ILD is not specifically defined in common stages like many cancers or kidney disease. Instead, providers typically classify progression of ILD into three phases:

  • Mild (early)
  • Moderate
  • Advanced (severe)

As a progressive inflammatory and irreversible condition of the lung tissue, providers can track deterioration of lung capacity to appropriately determine medical interventions. Phases can clinically advance through the stages at various paces outlined below:

  1. Early (Mild) Stage
    • Symptoms: The patient could experience shortness of breath during exercise or exertion such as climbing a set of stairs, or a dry cough.
    • Lung function: PFTs show mild impairment within the alveoli and resting oxygen levels can remain normal.
    • Impact: In this stage of the disease, the patient experiences minimal disruption to everyday daily activities.
  2. Moderate Stage
  • Symptoms: Increased feeling of breathlessness may occur with very minimal exertion, or even at complete rest. Due to implications of stiffening and fibrotic changes in the lungs, persistent cough, fatigue, and weakness in the patient become more pronounced.
  • Lung function: PFTs reveal a moderate decline in lung capacity.
  • Impact: This stage could include reduction in tolerance to exercise, leading to decreased ability to perform normal activities of daily living (ADLs).
  1. Advanced (Severe/End) Stage
  • Symptoms: Tasks which used to be routine become extremely difficult to complete. Severe shortness of breath develops due to chronicity of lowered oxygen levels in the blood, leading to clubbing in the fingertips. Patients at this stage often develop frequent flares, leading to further decline of pulmonary function.
  • Lung function: In this stage, lung functions are severely compromised, leading to drastic oxygen level deterioration.
  • Complications: During this stage, strain on the lungs and heart can lead to serious underlying conditions, such as pulmonary hypertension (high blood pressure in the lungs) and eventual respiratory failure. 

Remember These Coding Tips and Tricks

A frequent and significant pitfall in ILD coding is concentrating only on the lung symptoms while overlooking the root cause. ICD-10-CM supports a combination coding approach to capture the specificity of the clinical condition more accurately.

Beginning with the notes of section J84.- (Other interstitial pulmonary diseases), the ICD-10-CM code book instructs you to capture other clinical conditions with the Code also note. At the same time, J84.- also features Excludes1 and Exclude2 notes with further instructions.

The Excludes1 note prohibits you from reporting a J84.- code with the following codes:

  • J70.2 (Acute drug-induced interstitial lung disorders)
  • J70.3 (Chronic drug-induced interstitial lung disorders)
  • J70.4 (Drug-induced interstitial lung disorders, unspecified)
  • J98.2 (Interstitial emphysema)

The Excludes2 note allows you to report a J84.- code with the following codes, if the conditions are occurring simultaneously in the patient:

  • J60 (Coalworker’s pneumoconiosis)
  • J61 (Pneumoconiosis due to asbestos and other mineral fibers)
  • J62.- (Pneumoconiosis due to dust containing silica)
  • J63.- (Pneumoconiosis due to other inorganic dusts)
  • J64 (Unspecified pneumoconiosis)
  • J65 (Pneumoconiosis associated with tuberculosis)
  • J66.- (Airway disease due to specific organic dust)
  • J67.- (Hypersensitivity pneumonitis due to organic dust)
  • J68.- (Respiratory conditions due to inhalation of chemicals, gases, fumes and vapors)
  • J69.- (Pneumonitis due to solids and liquids)
  • J70.- (Respiratory conditions due to other external agents)

When ILD is due to an autoimmune disease, then sequence the connective tissue disease code first, followed by the appropriate ILD diagnosis code.

Environmental antigens can also be referred to as hypersensitivity pneumonitis (J69.0 [Pneumonitis due to inhalation of food and vomit]). Additional specificity codes include relevant external agent codes (for example, J70.5 [Respiratory conditions due to smoke inhalation]).

When the provider detects ILD along with medication as an adverse effect of a drug, you’ll assign two separate codes. Start with the specific lung condition as the primary code (for example, J84.9 [Interstitial pulmonary disease, unspecified]) followed by the appropriate code from the T36-T50 codes, “Poisoning by, adverse effects of and underdosing of drugs, medicaments and biological substances.” Use the 5th or 6th character to specify the drug.

Remember — specificity is key when coding ILD. Always review the provider’s documentation for the exact type, cause, and stage of the disease. ICD-10-CM ILD codes often require you to identify whether the condition is idiopathic, related to an underlying disease, or caused by environmental exposure. Pay attention to terms like “fibrosis,” “hypersensitivity pneumonitis,” or “connective tissue disease-related ILD,” as these guide code selection. Query the provider if documentation is unclear or incomplete. Using coding guidelines in conjunction with clinical notes ensures accuracy, supports medical necessity, and reduces claim denials, while improving both compliance and patient care outcomes.
