Document the Complete Clinical Picture Beyond Chest Pain
Consider these best practices for documenting diagnostic evaluations, supporting medical necessity, and improving coding accuracy. Chest pain is among the most common presenting complaints in emergency departments (EDs), accounting for millions of visits annually. Diagnostic evaluation involves clinical assessments, laboratory studies, imaging, and risk stratification to determine the underlying cause and appropriate level of care. Because of this complexity, accurate and thorough clinical documentation is essential for accurately portraying the patient’s clinical condition, supporting appropriate code selection, demonstrating medical necessity, and facilitating reimbursement. Use the following tips to strengthen your documentation and improve coding outcomes. Prioritize Specificity to Tell the Whole Story Chest pain is a symptom that may be associated with various conditions, ranging from benign disorders to life-threatening emergencies. While cardiac etiologies are often prioritized during a diagnostic evaluation, differential diagnoses can also include gastrointestinal, pulmonary, musculoskeletal, neurologic, or psychological conditions. Since chest pain may be a manifestation of numerous disease processes, documentation should clearly reflect the evaluation, medical decision making (MDM), and final assessment of the patient’s condition. Specificity provides a clear clinical picture of the patient's condition and serves as the foundation for accurate code assignment. The ICD-10-CM Official Guidelines for Coding and Reporting, Section I.B.2, states that you should assign diagnosis codes to the highest level of specificity supported by the medical record. While coding guidelines permit reporting “probable,” “suspected,” or “rule out” conditions, the outpatient setting and professional services require a definitive diagnosis. In the absence of a definitive diagnosis, the same principle of specificity applies to reporting signs and symptoms. Best practice is to document the precise location, character, and any other associated symptoms rather than using the nonspecific term “chest pain.” Link Test Results to Definitive Diagnoses The first priority in a diagnostic evaluation of chest pain is ruling out life-threatening conditions. This may include laboratory studies, imaging, and procedures. Linking diagnostic test results, such as troponin levels and other serial laboratory studies, to the provider’s clinical assessment and documenting their significance is essential to establishing and supporting a definitive diagnosis. Elevated troponin values alone do not establish myocardial infarction or myocardial injury, and normal results do not allow coders to assume a condition has been ruled out. Providers should document the clinical significance of all test results to support the final diagnosis. Coverage for many laboratory tests, diagnostic studies, and procedures depends on specific documented clinical indications. Ensure Documentation Reflects Guideline Requirements The diagnostic evaluation may require that the patient be admitted to observation or hospital inpatient status. Observation services are provided to patients who require a period of monitoring or treatment while a determination is made as to whether they may be discharged or their condition necessitates an inpatient admission. These decisions are typically made within a 48-hour timeframe. Observation services include ongoing short-term treatment, assessment, and reassessment. The record must contain dated and signed orders regarding services the patient is to receive, nursing notes, and progress notes. For inpatient services, the patient typically has a condition that directly threatens life and safety if treated in a less invasive setting. It is expected that they will require care spanning at least two midnights. A formal order must exist, and documentation must support medical necessity. Supporting higher levels of care begins with thorough documentation that reflects the severity of the patient’s condition. Clearly documenting a more specific diagnosis, such as cardiac chest pain or angina rather than simply “chest pain,” may influence whether a patient meets medical necessity criteria for hospital admission or observation. Avoid These Common Documentation Gaps One of the most common documentation gaps is the overuse of R07.9 (Chest pain, unspecified). This can be due to incomplete documentation, unclear diagnostic conclusions, lack of a definitive diagnosis, or cloned documentation in which the final assessment and diagnoses have not been updated. This can potentially misrepresent the severity of the patient’s illness, reduce the accuracy of quality metrics, and make it difficult to measure outcomes for cardiac and noncardiac etiologies. Frequent overuse may also impact reimbursement as it can trigger denials, delayed payment, and payer audits. Many of the more extensive diagnostic procedures that are part of the evaluation have coverage requirements that do not support the use of unspecified codes. Another common gap is the documentation of risk factors. Documentation of relevant risk factors can make the difference in low-, intermediate-, or high-risk designations. Risk factors, in addition to the patient’s presenting symptoms and clinical picture, determine the clinical decision pathway the workup will take. Patients with intermediate- or high-risk cardiac factors will benefit from more extensive testing, but that risk must be supported by the documentation. Conditions such as hypertension, diabetes mellitus, hyperlipidemia, tobacco use, obesity, coronary artery disease, prior myocardial infarction, or significant family history are examples of risk factors that may influence the steps taken in the diagnostic workup. Documentation for the diagnostic evaluation of chest pain is inherently complex. Comprehensive documentation is essential to support the complexity of coding, distinguish cardiac from noncardiac etiologies, justify observation or inpatient services, and avoid reliance on nonspecific diagnoses when more specific information is available. Ultimately, complete and specific documentation improves coding accuracy, reimbursement, quality reporting, and the overall integrity of the medical record.
