  Answer: Code 31615 (tracheobronchoscopy through established tracheostomy incision) would be the most appropriate for a flexible bronchoscopy via a tracheostomy. This code includes both surgical work (removal of obstructing crusts) and diagnostic bronchoscopy performed by the same physician. If photodynamic therapy is used to remove stenosis or tumors causing a blockage of the airways, it would be appropriate to report 31641 (bronchoscopy, [rigid or flexible]; with destruction of tumor or relief of stenosis by any method other than excision [e.g., laser]) and 96570-96571 as appropriate.
