The lab analyst measures both the total and free carnitine on serum, plasma, urine, or tissue specimens. Total carnitine includes both the free carnitine and all the acylcarnitines. Carnitines are important carriers that shuttle long–chain fatty acids into the mitochondria where the fatty acids are used as cellular fuel. Practitioners may order carnitine testing when a newborn screen tested positive on the blood spot test.
