I work for a Pain Management doctor & have a wondering what CPT should I use for SOAPP/COMM, CESD-R, PMQ-R, CPCOG? The office has been using G0444, 96136, & G0396, but I am having problems getting these codes paid. I have tried to research these codes to see if there are better CPT codes but cannot find anything. I am new to the pain management world, so these codes are foreign to me. Any help would be appreciated!

Facet Joint Injections64490 – 64492
Nerve Blocks64405, 64450, 64493 – 64495
Radiofrequency Ablation64633 – 64636
Spinal Cord Stimulation63650, 63685

20552: Trigger point injection for 1-2 muscle groups.

64405: Greater occipital nerve block.

20610: Joint aspiration or injection for conditions like osteoarthritis (M19.90).

Use proper dx codes M54.50 M54.2 M54.6,M17-M19 DJD or knees, hips, joints just pain dx codes. If provider states in notes chronic pain add dx G89.29. Also Z dx codes can vary Z13.828 encounter for muscles & bones Also use modifiers of left, right and 50 if both sides. Add J code for meds and mg. amount on claim. If due to injury use dx S and Y dx codes plus episode of care and date of injury on claim. If referred from another doctor or provider add on claim too.

We received payment on some of the codes when we applied modifier 59, except for G0396.
