Ultrasound-guided tenotomy of the right elbow

The anatomy was again identified, the diseased tissue was visualized using ultrasound guidance and confirmed at the right elbow. The patient was placed in a prone position with the right elbow bent at 90° over the bed. The entire area was prepped to create a sterile field using ChloraPrep. A sterile sleeve was placed over the ultrasound transducer. Lidocaine 1% was administered using both a 25-gauge needle to create a skin wheal and a 22-gauge Quincke needle down to the pathologic tissue.

Once adequate anesthesia was confirmed, A #11 blade was used to make an incision through the skin, down through tissue layers, and down to the site of the pathologic tendon tissue. The instrument was introduced through the incision site advancing to the diseased tendon and tissue under ultrasound guidance. Once the needle tip of the instrument was confirmed to be on the pathologic tissue, the foot pedal was depressed to begin the resection, division, and excision of the intra-tendinous diseased tissue and calcifications along the length and width of the tendon and bursa until satisfactory changes were observed on ultrasound. The specific areas of treatment include the distal triceps insertion site.
