One of our providers is missing a code in a patient's chart from a date of service in October 2025. Do you know the specific timeframe allowed for us to amend charts to add a missing code?

Medicare simply says "timely". Generally, those kinds of requirements are organization-specific. Most hospitals expect charts to be complete or amended within 30 days. And if the code addition means that you're billing a corrected claim, the payer may have an opinion about that. If it's an administrative addition only, 2025 wouldn't be unreasonable.
