Consent for Treatment

Hoping this will be a rather straightforward question.

For reference, I am in NC at a specialist office (place of service is always 11, billed professional services)

How often do we need to get a consent for treatment signed? We do it at the first visit and then annually. This basically covers we have the right to see you and bill your insurance, etc.
We do in office procedures (not using any sedation) and we have patients sign an informed consent documenting the diagnosis, procedure, risks, outcomes, etc. But if we repeat that procedure, say we do one procedure on the right leg and a week later we do the same procedure on the left leg, do we need to have another informed consent signed? Or is the informed consent good for a set amount of time (1 year, period of treatment, etc)

Thanks!

We obtain consent for treatment annually. For procedures, these consents are obtained for each procedure. If a patient comes in for a procedure today, they sign an informed consent for that procedure. If the patient has multiple procedures the same day, we have them sign a consent for each procedure being done, since the indications, purpose, complications, side effects, etc. could all be different. If the patient comes back a week later to have the procedure repeated or done to a different site, another consent is obtained. Procedure consents have requirements. Here are a few links that might help.
