Annual Wellness Visits (Medicare)

I am an Auditing and Education Manager and am hoping someone may be able to point me to CMS guidance on this topic.

For the Medicare Annual Wellness Visit (AWV), when a cognitive assessment such as the Mini-Cog is performed (word recall, clock drawing, etc.), is there any CMS requirement that the actual clock drawing be scanned into or retained within the EMR?

I have reviewed the CMS AWV requirements and found that CMS requires detection of cognitive impairment through observation and, when appropriate, the use of assessment instruments, but I have not been able to locate any guidance specifically addressing retention of the completed clock drawing itself. CMS states that the AWV includes "detection of any cognitive impairment that the beneficiary may have through observation, consideration of concerns raised by the beneficiary, family member, caregiver, or others, and, if appropriate, through the use of assessment instruments."

If anyone has located CMS guidance, MAC guidance, or audit findings that specifically address whether the clock drawing must be scanned into the medical record versus documenting the assessment results and interpretation, I would appreciate the information.

When we perform MCR AWV we do save the mini-cog that was performed and scan it into the patient's chart.
