    WE SUBMITTED A CLAIM WITH 69205 RT AND 92502 LT INSURANCE DENIED THE 92502, ENCODER SAYS THAT A MODIFIER CAN OVER RIDE THE RELATIONSHIP BUT IF THE 92502 WAS DONE TO MAKE SURE THERE WAS A FB IN THE LT EAR THAT SHOULDNT BE CHARGED CORRECT?

    This seems like a simply question but I'm still second guessing myself. If the provider inserts an ear tube in the right ear under anesthesia and then examines the left ear, would it be coded as 69436-RT 92502-59-LT (or a X modifier depending on the payor). TIA
