    Wiki Dx in A&P

    Good Morning All, When we have a diagnosis that is documented in the A&P only the education team in my office is split on whether or not this is "dingable". I haven't been able to find any guidelines that says ya or nay on this topic. Any help with included resources/guidelines would help us...

    Wiki Assessment and Plan

    I have a few providers who consistently leave their assessment and plan out of their documentation. What is a good way to address this with them? I've provided them with several examples where they list the condition but there is nothing documented beside it and the HPI does not mention them...
