Wiki Diagnosis coding with H&B CPTs

Does anyone know where I can find the diagnosis coding guidelines for H&B codes? I can't seem to find them anywhere. (I understand the CPT codes, but we're getting lots of coding denials that are diagnosis-related. :confused:) Thank you! :)
