Wiki Accidental IUD removal???

IUD was accidentally removed during another procedure. It got hooked on a prep sponge. It was replaced. I'm drawing a blank on this. Should this be coded? Is there a modifier that should be attached indicating "accident". I know the codes for IUD insertion, but drawing a blank as to what to put...
