Wiki 40530 CCI edits

My physician billed for 40530 and 13152 and when I ran it thru our scrubber it kicked out for CCI as bundled. So I look up 40530 (Resection of lip WITHOUT RECONSTRUCTION) and under the excludes it states Excludes reconstruction (13131-13153)??? I am confused! Please help. Thank you for any...

Wiki New EBUS CPTs for interventional pulmonology

I have received two articles (ATS and AABIP both February issues) that state we can no longer use the 31629, 31628, 31633 or 31632 with the new codes... although there is not a CCI edit for them. Does anyone know more detail about the proper billing for these codes? CPT is not exactly clear...

i have a silly question, in the cci edits from medicare, when it says standard of medical/surgical practice does that mean you can add modifier 59 to the code in the second column, or if anyone knows where I can get a list of what they mean, thank you

Help, I have tried speaking with customer service and have now had to send a request to high up contacts. Has anyone noticed that Humana stopped paying for the Well visit when the patients have immunizations. They can give me no information on this. I've ran the codes and they do receive any...
