    I have been submitting claims for this patient using codes 64612 and J0585. Now Medicare wants a modifier on J0585. On line 1 J0585, 25 units $250, Line 2 J0585, modifier JW, 75 units, no fee. They are also saying we have invalid name, strength or dosage of the drug. Not sure what Medicare...
