Our physician attempted and failed a hysteroscopy with D& C. the procedure was aborted due to the suspected uterine perforation. This was done in a outpatient facility. Would you amend modifier 53 in this case?

When an OBGYN provider preforms a D&C or a hysteroscopy in the office, insurance carriers pay less than when performed in the hospital. Shouldn't it be the other way around? Is there an additional code or a different way to bill procedures in the office for a higher reimbursement rate? The cost...

Patient had a missed Abortion and we performed 59820 on 11/9/15. Patient had several episodes of heavy bleeding following this. We then performed a Hysteroscopy D&C would I use 58558 with modifier 78 or do I use the 59160?
