Wiki Neuropsychology Diagnosis Coding vs Referring Physician diagnosis-what goes on the claim?

We have a neuropsychologist on staff and want to make sure we are coding the diagnosis list properly. Should the neuropsychologist only list the diagnosis codes founded in the evaluation or will it ever include diagnosis codes (founded or unfounded) from the referring provider?
