I am looking for the definition of full thickness. Dr. did a surgery stating, "We then debrided friable and necrotic tissue from around the hardware using Bovie electrocautery, Lexxel rongeur and Cobb elevator." Quiried him on the depth. i know this may sound like a dumb question but i just want to clarify....does the physician have to document "pressure ulcer" if they are documenting the stage? also when they are documenting a non pressure ulcer and saying it is partial thickness or full thickness is this ok? or do they have to...- sauls.amanda@yahoo.com
