    Wiki 99406 routinely denied when billed with E/M

    MCR is denying the 99213-99215 or 99203-99205 but paying the 99406 when billed on the same DOS. We are only linking F17.210 to 99406, while our other dx are linked to the 99203-99205 / 99213 - 99205 code. We are ensuring the provider documents all necessary elements in order to bill the 99406...

    Wiki Tobacco counseling billing

    UHC has new issue. They stated that tobacco use counseling covered under optim dx and recommended claim submission to rx. Does anyone already bill 99406 to rx? Thank you

    Wiki Proper use of 305.1 (Tobacco use disorder)

    When would you use 305.1 aside from when its specifically stated the patient is a smoker? If a pt had conjunctivitis, or sinusitis and the Health FX were checked with smoker, would this be coded? What conditions could/should this be used with?
