I am having difficulty getting this code paid under my local CMS contractor (Palmetto GBA). It keeps rejecting for invalid/missing modifier. I have tried anatomical modifiers as well as modifier 26 to no avail. Has anybody had success and, if so, can you provide some insight? Thank you, Valerie
