if a pt is having an EGD w BX for dysphagia and a screening colonoscopy turned BX .... Would you code 00813 PT ?
 
Screening colonoscopy with EGD (Colonoscopy versus EGD CPT code) and 00813, Code Z12.11 (plus applicable comorbid condition) and 45378 / 00813 for screening only. OR if becomes diagnostic/therapeutic procedure use PT modifier and secondary codes for established definitive diagnosis.
 
For 00813, we do not use a PT for a screening colonoscopy w/ a procedure w/ an EGD.

List Z12.11 first followed by the finding that necessitated the the procedure (ie, colon polyp), the diagnosis for the EGD, and a co-morbidity if one is provided. The co-morbidity will justify the use of anesthesia.

Example:
Patient has a screening with an EGD for dysphagia & a sigmoid polyp is biopsied. Patient also has COPD.
