Does anyone have experience with a Medicare Advantage plan (Preferred Care Partners) denying code 0408T (Insertion or replacement of permanent cardiac contractility modulation system), as "service is unproven and not covered." It is my understanding the Medicare covers this procedure, and Medicare Advantage plans must follow the same rules.
Any insight is appreciated.
