If a patient is in the hospital on 09/30, will they have to be discharged and readmitted with the ICD-10 diagnosis on 10/01 at midnight?
How does one handle this?

Interim billing, the same as you would bill for a really lengthy IP stay. Send one UB for admit date through 9/30, and a second for 10/1 through discharge, making sure to change the dx codes, bill types, etc.

All discharges on or after 10/1 are billed with ICD-10 for the entire inpatient stay, regardless of the admission date.
