CPT 11043 keeps denying by some insurance as incorrect place of service. We bill this with place of service 11 and some insurance will pay this and others are now denying. Is anyone else having issues with this? If so, what have you done? I have tried researching and I can not find anything on paper to be able to appeal this place of service.
